Initial visits: expect at least two, each lasting 90 minutes
Purpose of initial visits: evaluation and treatment planning
I will do my best to give helpful advice in the first visit.
Fee for each 90 minute visit: $600
After the initial visits, I will see you for 45 minute treatment visits. I find that 45 minutes rather than less time gives us an opportunity to get a fuller understanding of how you are doing and make decisions accordingly.

I believe in a collaborative approach. So I will welcome whatever information you would like to provide that may help me understand you better. And I will share with you the information you will need to make a well-informed decision. Ultimately, I will give you my best advice and together we will come up with a good, mutually-agreeable plan.
Treatment visits
Once the evaluation and treatment planning phase is completed (a minimum of two 90 minute visits), I will see you for treatment visits of up to 45 minutes. Depending on your particular needs, these visits are for medication management, talking therapy or a combination of the two.
For medication management visits, I like having 45 minutes rather than less time because it allows us to get a fuller understanding of how you are doing and to consider carefully what the next best step is. My advice regarding the frequency of these visits will depend on how you are doing. Fee for 45 minute visit: $475.

Private insurance
I have chosen not to be a member of any insurance company preferred provider panels. At each visit, however, I will provide a billing statement that you can submit to your insurance company. To determine whether you will receive benefits, you need to check with your insurance company; I do not have that information. If it is not possible to see me unless you are going to receive benefits, please check on this with your insurance company before calling me.

Paperwork
Before coming to the first appointment, there will be some paperwork to take care of. To access these forms, click on the Forms button. Bring the completed forms to our first meeting. Do not mail, fax or email them to me. If you are unable to have the forms printed, I can mail them to you.
Medicare beneficiaries
I have opted out of Medicare. This means that Medicare will not pay for my services; Medigap plans will not pay for my services; and other supplemental plans may not pay for my services.
